Community Nursing Symposium
A Continuum of Care: Supporting and Empowering the Community from Preventive Health to Disease Management

SingHealth Community Nursing adopts a person-centered care approach in bringing care into the community. Community Nurses from Singapore General Hospital, Changi General Hospital and Sengkang General Hospital work closely with hospital departments, primary care and community partners, and deliver broad-based generalist nursing services to help residents and patients keep well, get well and age well in the community. 

SingHealth Community Nurses support the transitional care needs of patients identified with risks of frequent readmissions through the multidisciplinary Hospital to Home programme. In recent years, they have also been involved in new initiatives to enhance post-discharge care and early discharge of patients, such as Telecare and Hospital@Home.

SingHealth Community Nurses have also, in collaboration with community partners, set up Community Nurse Posts, which are strategically located in Senior Activity Centres (SACs) and other community spaces in the Eastern region of Singapore, to support seniors in managing their chronic diseases in the community. They provide nurse-led services such as health and geriatric assessment, health coaching, education and support in chronic disease and medication self-management, as well as care referral and coordination for patients. They also provide generalist palliative care to patients with life-limiting diseases in the community. 

In addition, they regularly conduct outreach activities, such as targeted health talks and falls prevention screenings, to proactively reach out to seniors in the community. Since 2017, SingHealth Community Nurses have reached more than 20,000 residents.

SingHealth Community Nurses are expanding their services to include Women's and Children's health, starting with optimisation of preconception and maternal health in the community, as well as general child health support.

In this Symposium, Community Nurses will be sharing their care approaches, innovations and research in the areas of preventive care, disease management and maintenance to achieve better outcomes for residents across different population segments.
