Social prescribing comprises health, social and environmental dimensions to improve patient’s social, mental and physical well-being outside the hospital setting. This will help people to lead a heathier lifestyle and benefit from stronger community support. Hence, in this symposium, we aim to engage the torch bearers of experts in research of population health to enlighten us with the latest evidence and strategies in using social prescribing to effectively identify and support patients who face social risk factors that may affect their health outcomes.
