It is no longer acceptable for emergency services providers to “greet, treat and street” patients. Better outcomes are achieved through robust partnerships with family physicians, pre-hospital providers and community/ ILTC facilities. Through our presentations, we will share new, evidence-based ED models of care, transitional care strategies and collaborations with community providers to improve the health of our communities and those who may be vulnerable within residential facilities and prisons.
