This programme will be conducted in the format of several mini-lectures, sharing of the learnings from the implementation of social prescribing in primary care, with hands-on practice on attempting social prescribing on a hypothetical patient. The session will also illustrate how social prescribing can be the “social cure” by addressing the social factors that predict health outcomes for patients. Wellbeing Coordinators are the local equivalent of social prescribing link workers and SingHealth Polyclinics will share on how Wellbeing Coordinators can contribute to patient care.
