Asst Prof Jean Lee Mui Hua

Senior Consultant, Transitional Care and Community Medicine,
Sengkang General Hospital, Singapore


Asst Prof Jean Lee is a Senior Consultant Emergency Physician at Sengkang General Hospital (SKH). She is a faculty member of Yong Loo Lin and Singhealth-Duke NUS medical schools.

As Director of the new Department of Transitional Care and Community Medicine at Sengkang General Hospital, she is responsible for overseeing the development and progression of Transitional care and Community Medicine programs to right site health services and optimize healthcare capacity sharing in the Northeast of Singapore. Her outreach into nursing homes in the northeast and southeast of Singapore has seen substantial healthcare cost savings by enabling nursing home staff to practice at the top of their license, with care pathways and decision support protocols to manage the residents within the NHs, hence minimizing unplanned and inappropriate hospital admissions. She has won and led Agency of Integrated Care and Singhealth’s Population health Development multi-million dollar funds for this initiative.

As Director of Geriatric Emergency Medicine (GEM), she oversaw the setup of this relatively new discipline in SKH’s Emergency Department during the development of the SKH, where elderlies coming through the emergency department are screened for frailty and multi-disciplinary interventions implemented to optimize their wellbeing.

She is currently Co-chair of Singhealth’s Academic Clinical Program Geriatric Emergency Medicine(GEM) and as co-lead of the National Chapter for GEM Clinical Services, she helps oversee the powering of standardization of GEM practices across Emergency departments in Singapore.
