Ms Juweita Binte Arba'in

Nurse Clinician, Department of Population Health & Integrated Care Office - Integrated Continuing Care Services (PHICO-ICCS),
Singapore General Hospital


Ms Juweita is a Community Nurse Clinician, who has worked in SGH for the past 18 years.
She attained her Advanced Diploma in Gerontology Nursing in 2008, and bachelor degree from Curtin University in 2011. 

Ms Juweita has vast experience managing inpatient care, and in 2019, she was re-designated to Patient Navigator, performing care coordination and care transition role. 

In 2020, she was rotated to the Hospital to Home programme, and in 2021, she spearheaded the new Hospital at Home model of care in SGH. She is now leading a team of 5 nurses, and working collaboratively with a private healthcare provider for continuity of inpatient care at home. To date, the programme has enrolled more than 2000 patients, and saved more than 7714 hospital bed days.     

As Singapore faces a rapidly ageing population and hospital bed crunch issue, this new model of care “Mobile Inpatient Care” aims to address some of these issues and provide high hospital level care in the comfort of patients own home.     

Besides her heavy clinical load, Juweita is also a QI coach, Peer Supporter, Integrated Internal Auditor and Joy At Work department lead. She is passionate, resilient and well respected by her team.
