Asst Prof See Qin Yong is a consultant family Physician in the Care and Health Integration Department in Changi General hospital. He obtained his MBBS in 2009, Graduate Diploma in Family Medicine (GDFM) in 2014 and Master of Medicine  (Family Medicine) [MMed(FM)] in 2016 from National University of Singapore (NUS). He was later conferred the Fellow of College of Family Physicians Singapore (FCFPS) in 2019.  

Asst Prof See is involved in the transition and continuation of care of patients from hospital to home. Aside from providing consultation and support to the community nurses, he also conducts home visits for patients. 

Asst Prof See is also the Project Lead for Changi General Hospital’s Mobile - inpatient – Care@Home, an innovative, nationwide pilot model of care for patients to continue inpatient treatment in the comfort and familiarity of their own homes.
