Dr Tan is a Senior Consultant and Head of the Department of Family Medicine and Continuing Care (FMCC), SGH. FMCC is a key driver of transitional care services and innovative care models in SGH, in partnership with an extensive multidisciplinary team, hospital specialists and primary & community care providers, to facilitate to safe care transition of patients from hospital to home. Dr Tan is the Clinical Co-lead for the SGH@Home programme in SGH and has been involved in the set up and operations of SGH@Home since its inception.
