‘My Care Team’ (MCT), which has been rolled out in SingHealth Polyclinics, is a step towards achieving Healthier SG’s concept of ‘1 person, 1 doctor (1 team)’ in the context of preventive care. The aim of this symposium is to provide current evidence on team-based care and share insights gained from the process and outcome evaluation of the implementation of the ‘My Care Team’.
