A/Prof Low Lian Leng is Director, SingHealth Centre for Population Health Research and Implementation (CPHRI). CPHRI administers the PULSES Centre Grant and evaluates various national-level initiatives such as Healthier SG. Concurrently, he is also Director, Population Health and Integrated Care Office, Singapore General Hospital, Medical Director of Outram Community Hospital, and Chief Medical Informatics Officer, SingHealth Regional Health System. A/Prof Low is an NMRC HCSA recipient and is PI of national level grants on mHealth, AI for chronic disease management and social determinants of health. 

His team has implemented various population health programs in the community such as Community Nursing, Primary Care Networks and integrated community care teams through co-creating solutions with the community such as the People’s Association, social service agencies, Agency for Integrated Care and the Ministry of Health Office for Healthcare Transformation. A key enabler is the use of digital health solutions to enable data sharing and better care coordination.
