National Healthcare Innovation and Productivity (NHIP) Medal 2025

Organised by CHI and supported by the Ministry of Health as well as the Ng Teng Fong Healthcare Innovation Programme, the annual National Healthcare Innovation & Productivity (HIP) Medals is a prestigious, national-level award that recognises local and private healthcare; as well as community/ intermediate and long-term care institutions that have demonstrated thought leadership and excellence in healthcare innovation & productivity.

Excellence Champion Medal, BLAST and CLEAR Catheter in the Community, joint collaboration led by Dr Tan Ru Yu, Renal Medicine, SGH
In Singapore, many end-stage kidney failure patients depend on haemodialysis (HD), with tunnelled haemodialysis catheters (THCs) frequently used. However, THC dysfunction due to thrombosis is a common complication, traditionally managed in public hospitals. This approach often results in treatment delays, patient inconvenience, unnecessary hospital bed occupancy, and increased healthcare costs. The National Kidney Foundation (NKF) reported a median of 48 hospital referrals each month for THC-related issues – highlighting the urgent need for a more sustainable and patient-centric solution. To address this, two community-based initiatives—BLAST and CLEAR—were implemented. BLAST focuses on early detection and prevention by optimising blood flow through targeted anticoagulation, customised dosing, and standardised catheter care techniques. CLEAR empowers trained community dialysis nurses to administer thrombolytic agents directly at dialysis. 

Best Practice Medal (Care Redesign), Augmented Intelligence in Infectious Diseases (AI2D), A/Prof Andrea Kwa, Pharmacy, SGH 
Lower Respiratory Tract Infections (LRTIs) are one of the leading causes of hospital admissions and account for 21% of all antibiotic use—of which 30–40% is considered unnecessary. Diagnostic challenges, including the absence of rapid tests, reliance on clinical judgement, and fragmented data, often lead to precautionary overuse of antibiotics. Existing solutions like manual audits and rule-based Clinical Decision Support Systems (CDSS) are labour-intensive and have limited scalability. To address this, an AI-powered Clinical Decision Support System, AI2D, was introduced to optimise antibiotic use in LRTI cases. AI2D analyses clinical, laboratory, and imaging data to provide real-time, personalised recommendations on whether antibiotics are necessary. This enhances diagnostic accuracy and streamlines Antimicrobial Stewardship (AMS) workflows, enabling equitable, hospital-wide intervention with existing manpower.

The NHIP Automation, IT & Robotics Innovation (AIR) Best Practice Medal recognises exemplary projects that demonstrate outstanding innovation, scalability, and measurable impact in transforming clinical or operational practices. Awarded annually, it celebrates initiatives that harness technology to improve patient outcomes, enhance system efficiency, and strengthen healthcare resilience. In 2023, the AIR Best Practice Medal was awarded to the 4‑Dimensional Disease Outbreak Surveillance System (4D‑DOSS). This pioneering digital‑twin project integrates real‑time data, machine learning, and predictive analytics to identify at‑risk contacts and detect infectious disease clusters with speed and precision. By generating dynamic situational awareness and enabling earlier intervention, 4D‑DOSS exemplifies how data-driven tools can elevate public health capabilities and support safer care at scale.
