SNM Yvonne Lim is the Nursing Innovation and Transformation Lead at KK Women's and Children's Hospital (KKH). With more than 15 years of clinical experience as a paediatric oncology and wound ostomy nurse specialist, she brings along her vast clinical expertise to champion multiple innovative healthcare solutions. In her current role, she leads the KKH@Home nursing team in collaboration with a multidisciplinary healthcare group, pioneering hospital-level care delivery for women and children in their homes. Under her leadership, the programme has successfully established various care pathways such as home phototherapy that transform traditional hospital-based services into home-based care solutions.
