Advancing Hypertension Care in Singapore Primary Care

Despite having effective pharmacological and non-pharmacological treatments of hypertension, the prevalence of hypertension is rising, both globally and in Singapore. Half of the people with hypertension remain undiagnosed, while among those diagnosed, half remain uncontrolled. Our revent qualitative study involving doctors, nurses, pharmacists and patients identified challenges in diagnosis, self monitoring, interpretation of home blood pressure (BP) and adherence to medication. To address these challenges, it requires a change in practice i.e. less reliance on human behaviour and explore the use of technology. The paradigm of hypertension care must shift from merely controlling home BP to achieving 24-hour BP control and reducing variation in circadian BP.
