This panel explores the critical role of the social and environmental determinants of health in shaping patient outcomes, drawing on insights from community and hospital settings in Singapore. This discussion draws on case studies from AWWA’s Family Empowerment Programme and an upcoming poverty-health landscape analysis. It also includes initiatives at Tan Tock Seng Hospital that aim to increase healthcare staff’s awareness of non-clinical determinants of health, such as poverty and social isolation. Together, these examples highlight the need for healthcare professionals and systems to integrate social and environmental factors into clinical care.

Attendees will gain practical recommendations for how clinicians, healthcare professionals, and healthcare institutions can better identify and address non-clinical needs that affect clinical outcomes. Attendees will also understand how fostering stronger partnerships with community organizations can create a more integrated, holistic care ecosystem for patients inside and outside the hospital.
