A/Prof Lee leads SingHealth Community Health Office of Learning (also known as SCHOOL), SingHealth’s training unit for health–social care integration. A family medicine specialist trained in adult education and motivational interviewing, he incorporates these skills into programmes he developed, including transitional care and social prescribing. He pioneered transitional home care in SGH and introduced Social Prescribing in SingHealth Community Hospitals, now a national movement. 

A/Prof Lee is also a Senior Consultant at SGH focusing on complex patient needs. His leadership has earned major recognitions such as the National Community Care Excellence Team Award, the Smart Ageing Leader Award by the Golden Age Foundation, and the designation of SCH in 2024 as the world’s first WHO Collaborating Centre for Social Prescribing.
