A/Prof Lee heads SCHOOL (SingHealth Community Hospital Office Of Learning) which is the training unit for SingHealth Regional Health System that is focussed on competencies in health and social care integration. He is trained in adult education and motivational interview, and incorporate these into the curriculum that is developed by SCHOOL. He pioneered transitional home care in SGH in 2006 which eventually evolved into the hospital to home programme and other transitional care programmes.  In 2019 he introduced Social Prescribing as a further enhancement to care transition and navigation.  Other courses that he developed and taught include the WSQ Course in Implementing Health and Social Care Programmes, and Motivational Interviewing for Social Prescribing.  

He is a Senior Consultant at the Department of Family Medicine and Continuing Care in Singapore General Hospital and his practice is focussed on patients in complex health and social needs.
