Measuring Outcomes of Interprofessional Collaboration in the Continuum of Care for Population Health

Launched in 2023, Healthier SG is Singapore’s strategy to transform the way we deliver healthcare, by shifting our emphasis from reactively caring for those who are already sick, to proactively preventing individuals from falling ill. It aims to empower individuals to become healthier and improve their quality of life through preventive care and strong patient-primary care provider relationships, supported by community partners and the 3 healthcare clusters. 

Singapore will be heavily investing resources in mobilizing our network of family physicians, activating community partners and strengthening system enablers such as information technology, manpower development and financing policy to make Healthier SG work. Optimal collaboration between these partners through team-based interprofessional care (IPC) would support the delivery of quality population health across the continuum of care. However, the benefits of this team-based approach leading to tangible effect on healthcare outcomes lacks definitive proof. Without return on investment (ROI) evidence, healthcare leaders cannot justifiably throw their weight behind IPC for population health.

We propose using the Quadruple Aim framework to examine the ROI of IPC using four dimensions: patient outcomes, patient experience, provider well-being, and cost of care. A multimethod approach can be used to measure patient outcomes and healthcare cost using quantitative methods, and patient experience and provider well-being using qualitative methods.
