Advanced Practice Nurse (APN) Carolyn Chan is Deputy Director, Nursing from Sengkang General Hospital (SKH). She was certified APN in Community Health in 2009 and was practising at the primary care polyclinics in Singapore. The main area of focus is the management of patients with chronic diseases. She had published and co lead some research studies related to APN outcomes in the primary care and chronic disease management in the community. Since 2019 she was given the opportunity to lead and develop community nursing at SKH. In 2022, SKH was awarded the funding from Ministry of Health on a Maternal and Child Health project targeting at the low-income families in the North-East Region where Carolyn is the Clinical Lead.
