From July 2023, Healthier SG will help Singaporeans take steps towards better health. This strategy shifts the country's healthcare focus from reactive care towards preventive care, which focuses on healthcare outcomes that tackle illness at its roots before it progresses to the point of needing treatment.

As part of SingHealth Polyclinic's efforts to build a Healthier Singapore, our team-based care programme, My Care Team (MCT), allows patients to receive integrated care from a multi-disciplinary healthcare team, consisting of Doctors, Care Managers and Health Pals, who strives to transform patient experience and actively engage and empower patients as full participants in preventive care.
MCT piloted at Eunos and Punggol Polyclinic in July 2022, and the programme was officially rolled out to all SingHealth polyclinics by January 2023.
Madam Tee, a patient from Punggol Polyclinic, was among the many who was happy to be a part of MCT, "I feel more comfortable knowing that I can get to see the same team of healthcare professionals during up follow-up visits at the polyclinic. I have also recommended my friends to join as I see the benefits of having a dedicated team taking care of all my healthcare needs."
Adopting a preventive care approach, MCT harnesses the strengths of each member and pools knowledge and resources to develop a personalised and comprehensive care plan for each patient. As Health Minister Ong Ye Kung shared, "Prevention is better than cure." MCT promotes preventive care by encouraging patients to go for appropriate health screenings as well as vaccinations. With proactive follow-ups to address preventive or chronic care gaps, patients will be able to better navigate their care plans and take ownership of their own health.
